“A veteran with chronic heart and lung disease wanted to avoid moving into residential care. Through DVA community nursing, Gentle Care provided regular visits for medication management, wound care, and monitoring of breathlessness and fluid changes. We kept their GP informed and helped the family understand when to seek extra review. With this support, they were able to stay at home comfortably for much longer than they expected.”
