Case example: Stabilising support for an NDIS participant at home
“A participant in their 30s with complex physical needs was cycling through hospital admissions because daily care at home kept breaking down. Their support coordinator referred to Gentle Care for more consistent nursing. We worked with the participant, family, and therapy team to set up regular in-home nursing, safer manual handling routines, and clear escalation steps. Over the next few months, avoidable admissions dropped and the participant felt more in control of their week.”
